Proof of Loss Declarations Sworn by a Vancouver Commissioner of Oaths

If your car was stolen, or your house broken into, burned or flooded, you'll need to deal with your insurer immediately. You will need to file a claim with your insurance company to recover your losses. Once you have contacted your insurer, your claims adjuster will likely ask you to complete a proof of loss form and to get it signed before a B.C. Commissioner of Oaths. There are various types of proof of loss forms, but what they all have in common is that you (the insured) is making a solemn declaration as to the nature of the loss. For any of these forms, you need to see a Commissioner of Oaths.

Proof of Loss Forms

The Proof of Loss form that you will need to sign is provided to you to comply with the Insurance Act. You will notice that most Proof of Loss forms will say 'I.B.C. Claim Form' at the top.  These forms are prepared by the Insurance Bureau of Canada. The IBC was established in 1964, and is the national industry association representing Canada's private home, car and business insurers. Its member companies represent 90% of the property and casualty (P&C) insurance market in Canada. IBC Claims Forms are developed by IBC's Claims Committee to help standardize claims reporting.
